UCLA employees may choose receive their W-2 forms either online or in paper.On-line distribution is available on At Your Service On-line by the third week of January. For directions to set up an AYSO account please visit http://map.ais.ucla.edu/go/1004679 or call the AYSO hotline at (800) 888-8267. Please note, you will need internet access at the time of set up. Paper copy distribution is mailed out by January 31st of each year to the address of record as of the middle of January. Undeliverable W-2 forms may not be forwarded by the United States Postal Service; they are usually returned to Payroll Services. An electronic copy will become available on AYSO in mid February.
Employees can also request a duplicate W-2 by submitting a Duplicate W-2 Request Form to payroll services.
