The information you provide will be included in the 2012-13 UCLA School of Law Student PhotoBook along with your picture. Our goal is to make the publication available to law school students, faculty and staff early in the fall semester.

Carpool Information Exchange Form

Law students who live in a valid carpool area are guaranteed a carpool permit at a discounted rate provided they apply by the posted deadline.  For assistance in forming carpools, the law school provides a secure site on-line where law students may exchange information; contacts and arrangements are the responsibility of the individual student. 

Family Information Form

Completion of this form is voluntary; however, the school appreciates receiving the requested information.  It will be used by UCLA School of Law External Affairs to invite families to participate in UCLA School of Law events and programs.

Roommate Information Exchange Form

To reduce housing costs, students often choose to share an apartment with other law students.  To assist law students with their roommate and vacancy search, the law school provides a secure site online for law students to exchange information; contacts and arrangements are the responsibility of the individual student.  After you have completed the Roommate Sign Up form, click on the “Roommate Search” link on the left. If you have found a roommate and you would like to opt out of the database, click on the “Roommate Opt-Out” link on the left.

Student Mentor Request Form

A group of second and third year law students has volunteered to be mentors to the incoming class.  Our plan is to have a social event during the first full week of school to provide new students and their mentors an opportunity to get together informally.  If you would like to participate in this program, please complete the form online by the deadline.  We will attempt to match you with a current law student of similar background or interests and you will receive the name of your assigned mentor during Orientation.  The information you provide will be shared with your student mentor.  Stay tuned as well for information about other mentoring opportunities connected with programs and student organizations.
