In your request, please include: your name, signature, UCLA ID # or the last four digits of your Social Security #, date of birth, year of your graduation, a contact phone number in case we need to reach you, and the address to which the diploma should be sent. Diplomas are sent via certified mail with a recipient signature required for delivery. There is no cost for mailing.
You can authorize a trusted third party to pick up your diploma by sending in advance a written, signed request to the Records Office. In addition to the usual diploma request information (your name, signature, UCLA ID # or Social Security #, date of birth, year of your graduation, a contact phone number), you must include the name of your designate and they must present their own photo I.D. when they come for pick up. Authorizations can be faxed or scanned and emailed in to the Records Office.
Your diploma is not released if you have outstanding financial obligations to the University (you can check URSA for information on any outstanding holds on your academic records).
REPLACEMENT DIPLOMAS
If your original diploma has been lost or significantly damaged, you can request a replacement diploma. In addition to the ordering process outlined above, you must include information regarding the status of the original diploma (e.g., whether it was damaged or lost) and a check to cover the $60 fee for a replacement diploma. It will take approximately 8 weeks to reprint a replacement diploma. The replacement diploma will bear a reissue date and the signatures of the current officials of the state and University.
